Key Responsibilities
- Coordinate all Swing Bed admissions, transfers, and discharges
- Serve as the primary liaison between patients, families, providers, and care teams
- Complete utilization reviews and ensure regulatory compliance
- Track outcomes, maintain documentation, and support quality improvement
- Promote the Swing Bed Program to referral sources and community partners
Qualifications
- RN or LPN preferred; relevant clinical or case management experience preferred
- Strong communication and patient-advocacy skills
- Knowledge of Medicare Swing Bed regulations a plus – training will be provided
- Ability to work independently in a rural hospital setting
- Excellent communication and coordination skills
- A heart for rural healthcare and patient-centered service
